ADMISSIONS

The caring and compassionate professionals of East Neck's Admissions team provide guidance to individuals seeking placement in our sub-acute care and rehabilitation and skilled nursing programs. They also assist with discharge planning to assure safe return to the community.

Each prospective patient and/or their family members will be given a thorough explanation of the admissions process, a pre-admission interview and a tour of the facility. Admission forms must be completed by the patient and his/her family or sponsor prior to admission to the facility.

East Neck Nursing & Rehabilitation Center strives to be your home away from home and our Admissions Department will make every effort to assist with admissions procedures and address any questions and/or concerns you may have during the process. We also invite you to contact us to schedule a tour of our facility.

Eligibility:

  • Applicants with a minimum age of 18 years who qualify for nursing home placement, sub-acute care or rehabilitation will be considered for admission.
  • East Neck requires a completed application, Patient Review Instrument (PRI) and Screen prior to admission.
  • In addition to private payment, we also accept most private insurance plans, including:
    • Medicare
    • Managed Medicare Programs
    • Private Health Insurance
    • Health Maintenance Organizations (HMOs)
    • Medicaid 

If you are interested in Medicaid coverage, our on-site Medicaid Coordinator will assist in determining your Medicaid eligibility and will initiate the application process. Please contact our Admissions Department at (631) 422-4800 for assistance and to determine if we are in network with your insurance plan.

Our Admissions Process     
The first step for consideration into a Skilled Nursing Facility is the completion of a Patient Review Instrument, or PRI. Once completed the PRI acts as an assessment tool and is forwarded to our Admissions Department for review. Referrals may be initiated by Hospital Discharge Planners, Community Organizations or other Skilled Nursing Facilities.

East Neck's Admissions Department handles all inquiries and can be reached by calling (631) 422-4800.

What to Bring
We ask that all patients please bring a two-week supply of clothes (including sleepwear) with them upon arrival to the facility.  Our Laundry Department will label all clothes with names to ensure they are returned to their owners.  Please bring any new clothes to our reception area so they can be forwarded to the Laundry Department for labeling.

We supply personal hygiene items for your convenience. If you have preferred brands or products, please feel free to bring them with you.

For added comfort, you may bring a portable television, radio or alarm clock to furnish you room with. We can also provide each patient with a rental television if needed.  If you have a favorite chair or recliner, ask us about accommodating it in your room.

Upon Arrival
Upon arrival at East Neck, you will be greeted by the Director of Admissions or her assistant.

Welcoming Patients, Visitors and Friends

Welcoming Patients, Visitors and Friends At East Neck, we perform in-house laboratory testing that often eliminates the need to send patients outside of the facility for necessary medical tests. We routinely screen patients for strep throat, mononucleosis and influenza and perform basic blood sugar, urinary and other laboratory testing as needed. Our practitioners will collect blood samples and send them out to outside laboratories for tests we do not perform in-house. For patients requiring x-ray imaging, East Neck conveniently performs in-house radiologic imaging using our digital x-ray system.
