Hospice care provided by a dedicated and compassionate team can make a meaningful difference for those facing end of life, and for their loved ones. At East Neck Nursing and Rehabilitation Center, we coordinate care and services with experienced hospice organizations to provide the highest quality hospice care to patients. Skilled and sensitive hospice professionals assist patients with physical pain management and address the emotional and spiritual needs of both the patient and family.
We contract with several hospice agencies that provide exemplary hospice services to those patients in need of individualized end of life care. Once a decision is made for hospice services, the staff at East Neck will arrange a meeting with the patient, their family and the hospice agency to discuss the array of hospice services to be provided at our facility.
As a collaborative team, both East Neck and hospice staff develop an individualized plan of care, focusing on the holistic needs of the patient, with comfort being a main objective. We encourage family members to attend frequently scheduled meetings. The staff at East Neck understands the impact the imminent of death of a loved one places on family members and strives to also provide emotional, educational support and counseling to family members.

Welcoming Patients, Visitors and Friends
At East Neck, we perform in-house laboratory testing that often eliminates the need to send patients outside of the facility for necessary medical tests. We routinely screen patients for strep throat, mononucleosis and influenza and perform basic blood sugar, urinary and other laboratory testing as needed. Our practitioners will collect blood samples and send them out to outside laboratories for tests we do not perform in-house. For patients requiring x-ray imaging, East Neck conveniently performs in-house radiologic imaging using our digital x-ray system.
